Scoliosis

Scoliosis is a lateral curvature of the spine affecting an estimated 2–3% of the population, with adolescent idiopathic scoliosis (AIS) being the most common form. While mild curves may only require observation, moderate curves (typically 25–40° Cobb angle) in growing patients respond well to bracing — one of the most rigorously studied non-surgical interventions in orthopedics. For adults, bracing can reduce pain and improve functional posture.

Common Causes of Scoliosis

• Adolescent idiopathic scoliosis (AIS): most common; cause unknown
• Neuromuscular scoliosis: caused by cerebral palsy, muscular dystrophy, or spina bifida
• Congenital scoliosis: vertebral malformation present at birth
• Degenerative scoliosis: adult-onset from disc degeneration and vertebral changes
• Scheuermann’s kyphosis: hyperkyphosis with vertebral wedging in adolescents

Common Symptoms:

• Uneven shoulder or hip heights
• One shoulder blade protruding more than the other
• A visible curve in the back, especially when bending forward (Adam’s forward bend test)
• Uneven waistline or clothing fitting asymmetrically
• Back pain and fatigue with prolonged standing (adult scoliosis)
• In severe cases: cardiopulmonary compromise from thoracic curve

How Is Scoliosis Treated?

• Observation for mild curves (less than 25°)
• TLSO scoliosis bracing for curves of 25–40° in skeletally immature patients
• Physiotherapy scoliosis-specific exercises (PSSE): Schroth, SEAS methods
• Surgical spinal fusion for curves exceeding 45–50° or progressive curves
• Bracing in adults to relieve pain and prevent progression
• Custom vs. prefabricated TLSO options based on curve pattern

How a TLSO Scoliosis Brace Works:

The landmark BRAIST trial published in the New England Journal of Medicine demonstrated that TLSO bracing successfully prevented curve progression and reduced the need for surgery in adolescent patients with scoliosis — with success rates directly correlated with hours worn per day.
