Please print this application form and fill in the requested information to become a registered buyer with RCAI. Fax it to us at + (727) 573-1886 or toll-free fax + (800) 545-7938 along with your tax exempt forms.

You will be contacted by RCAI when your account is ready to use. Please note that you must fill in all the requested information in order for us to process your request. All information will be held in the strictest of confidence.

On your purchase order form:

1. Specify your purchase order number
2. Specify the product code or product description and size you wish to order
3. Specify the quantity you wish to order
4. Specify the address where the order is to be shipped
5. Specify how you want the order to be shipped to your facility.

   Call the order to +800-627-1595 or +727-573-1595.
7. You will receive an order confirmation on faxed or e-mail orders.
